The consultation form is a simple contact form. It isn't a HIPAA-covered patient portal. Please don't send detailed medical records, test results or insurance numbers through the form. You can go over your history and imaging with the doctor at your consultation. Once you become a patient, your medical records are handled by Stapel Family Chiropractic under its own patient privacy practices.

The site doesn't use advertising cookies. Our hosting provider may use strictly necessary cookies or similar technology to keep the site secure and working. Embedded maps are provided by Google and are subject to Google's own privacy policy.
Keeping it safe
